In the omni sanctuary quest in the part where Roch is summoned if you fail to get your pattern defiled before he despawns you cannot resummon him because you now have the wrong mission (speak to Roch). This happened to me because of a crash and i belive the only way to fix it is restart the quest. I'm not sure if this counts as a bug or not but it's sure bugging me.
